A Nurse Practitioner (NP) Lead in Cognitive Care manages complex dementia/memory disorder care, leading interdisciplinary teams to create & implement personalized plans, diagnose/treat cognitive decline via assessments (MMSE, MoCA) & meds, educate caregivers, and coordinate community resources, focusing on holistic, evidence-based care for patients with Alzheimer's, Lewy Body, etc., often using telehealth/integrated models. This role requires advanced NP skills, strong leadership, excellent communication, and managing both clinical escalations and caregiver support to improve outcomes and reduce burden.
Key Responsibilities
- Clinical Patient Management:
- Conduct comprehensive cognitive/physical assessments, medical history reviews, and interpret labs/imaging.
- Diagnose and manage cognitive impairment, dementia (Alzheimer's, FTD, LBD), and behavioral symptoms.
- Develop, implement, and monitor individualized, person-centered care plans.
- Provide palliative/end-of-life care planning and support.
- Leadership & Collaboration:
- Serve as a clinical escalation point for the care team.
- Lead and coordinate with neurologists, geriatricians, behavioral health, social workers, etc..
- Mentor staff, foster a culture of safety, quality, and professional growth.
- Patient/Caregiver Support:
- Educate families on diagnoses, treatments, disease progression, and coping strategies.
- Provide behavioral management coaching and resources to reduce caregiver distress.
- Program & Quality Improvement:
- Implement evidence-based practices and stay current with dementia care guidelines.
- Utilize data for care plan review, quality improvement, and research.
- Operational/Administrative:
- Accurate EHR documentation, compliance, and managing resources efficiently.
- Participate in on-call rotations and coordinate with community resources/discharge planning.
Essential Qualifications
- Master's (MSN) or Doctorate (DNP) in Nursing.
- Active credentials with major insurance payers throughout Texas.
- Active, unencumbered RN & APRN licensure in the state.
- Certification (e.g., PMHNP or Gerontology NP) is often preferred.
- 2-3+ years in neurology, dementia, or geriatric care.
- Strong clinical judgment, leadership, communication, and empathy.
- Experience with telehealth, integrated care models, and managing complex cases.
